Student Grants Documentation
Here are some of the things you're most likely to need to include with the application form. If, like most first-time students, you are under 23, please note that items 2 to 5 relate to both your parents and you.

Full Birth Certificate of the person who's going to college/university – if you’ve lost it, you can get it from the Registrations Office, County Clinic, Mullingar, 044 9395098, or online from the Civil Registration Service;

If you’re employed, a P21 Balancing Statement form for 2009, from the Revenue office, Pearse St. Athlone, or by ringing 1890 777425, or at www.revenue.ie (registration required);

If you’re employed, your P60 for 2009, and/or any P45 you got if you left a job in 2009;

If you are self-employed, e.g. a farmer or tradesperson, you need to get; (a) your Notice of Assessment for 2009, from the Revenue office, Pearse St. Athlone, or by ringing 1890 777425, and; (b) your Accounts for 2009;

If you receive any payments from Social Welfare, contact the social welfare office/department that pays you and ask them for a letter stating the type of payment it is and how much in total they paid you in 2009.
